Mafia: The Old Country is a prequel entry in the long-running Mafia crime saga from Hangar 13, published by 2K Games, released August 8, 2025 on PC, PlayStation 5, and Xbox Series X|S. It's a third-person, primarily linear, narrative-driven experience set in the fictional town of San Celeste and the wider Valle Dorata region of 1900s Sicily, returning to the structured, cinematic storytelling of the earliest Mafia games rather than the fully open-world approach of Mafia III.

The Story

You play Enzo Favara, whose childhood was spent in indentured labor within Sicily's brutal sulfur mines before he's drawn into the world of organized crime, eventually joining the notorious Torrisi family. By swearing an oath, Enzo commits himself to the Torrisi family's code of honor -- with all the power and hardship that entails -- as the game explores themes of family, loyalty, and sacrifice amid violent power struggles between rival mob factions.

Reception

The base game holds a Metacritic score of 73/100, with critics generally praising its narrative, characters, authentic Sicilian setting, and visual fidelity. Beyond the core narrative campaign, a separate Free Ride mode lets players explore the Sicilian countryside at their own pace or take on repeatable Challenges outside the critical story path, including combat encounters and stealth missions.

Man of Honor: A Villain Origin Story

Announced at Summer Game Fest 2026 and releasing August 14, 2026 for $10 on PS5, Xbox Series X|S, and PC, Man of Honor is the base game's first paid expansion -- and it's a genuine character-origin story for one of the Mafia franchise's most iconic villains. Set in Sicily during the winter of 1905, the expansion follows a now-proven Enzo Favara and his associate Cesare as the Don assigns them to assist Ennio Salieri, a man of honor recently released from prison and intent on reclaiming what's his. Longtime series fans will recognize Salieri as the powerful Don and central antagonist of the original 2002 Mafia and its 2020 remake, Mafia: Definitive Edition -- the crime boss whose organization protagonist Tommy Angelo works for and ultimately turns against, decades after the events depicted here. Man of Honor depicts a younger, scrappier Salieri years before his emigration to America and eventual rise to power in Lost Heaven.

What Man of Honor Adds

The expansion delivers two new story chapters continuing Enzo's early days as a soldato, alongside fresh environments, weapons, vehicles, and Charms. Owning Man of Honor also unlocks exclusive new Free Ride mode content starring Enzo, with Salieri acting as a new point of contact -- including a fresh set of exclusive jobs, extreme challenges, and new collectibles and hidden locations added directly to the Valle Dorata map. Notably, even players who purchased the $59.99 Deluxe Edition of the base game at launch will need to purchase Man of Honor separately, as it isn't automatically included.
